[SATLUG] Any recommendations for Change Management Software?

Do you all have any recommendations for change management software?  Here
are my requirements:

*  Web-based control panel
*  Not too complicated.  Some of these systems are so hard to use it takes
longer to do the CRQ than to actually perform the work.
*  Prefer FOSS, but will consider paid packages if they are a good fit.
