I'm going to piggyback in on Al's reply. I seem to have misplaced the original.
What you seem to be looking for is a package that someone is using
that fits what you are doing. To explain what you are doing would take
some time, and like all of us, there are some fuzzy parts that are
hard to explain.
Here's quote from Wikipedia:
http://en.wikipedia.org/wiki/Change_management_process
"There is considerable overlap and confusion between change
management, change control and configuration management."
Yea, Buddy!!! Just put it on your resume and watch the confusion.
Every potential employer has a "product specific" definition.
I would suggest you look hard at a wiki for a low end approach.
Places to start looking are:
http://en.wikipedia.org/wiki/Comparison_of_wiki_software
"ITtoolboxwiki - Change Management"
http://wiki.ittoolbox.com/index.php/Change_Management
Mark McCoy of SATLUG is using dokuwiki, but not as a Change Management tool:

http://wiki.splitbrain.org/wiki:dokuwiki
http://en.wikipedia.org/wiki/DokuWiki
The high end approach would be to look at a CMS.
Several SATLUG contributors recommend:
Check out and compare all of the most popular ones here:
http://www.opensourcecms.com/
Here is an actual working example of a wiki used to manage an ILM
(Information Life-cycle Management) project. This is a very high end
approach.
http://ilm.twoday.net/
Change Management, Change Control and Configuration Management are
extremely important Lower Metric processes in a project like ILM. They
have to work the same way every time (Reliability) and produce the
correct results (Validity).
Be very interesting to see what you finally choose and use.
