While I agree with the sentiment, I have had more problems with Linux
sound systems than Windows.

Could reinstall Windows, but I am sure that you would rather not do that.

I have just replicated an issue on my Dell D820.  Unlike yours, it
does not seem to be app specific.  I am going to reboot now, and see
what happens....

It appears that my particular problem was the check box "PC SPK Mute"
being checked.  External speakers worked but my internal speakers did
not.  I doubt this will help you, but FWIW the checkbox is in the
Advanced section under the "Adjust Audio Properties" context.
